HOW TO ENROLL

Our practice is open for new patients.
We invite you to join. 


Please read our website for all information about our philosophy, practice and fees. If you need an enrollment packet by mail, call us at 415-380-8448.

Secret Garden Membership is required to be a patient at Pediatric Alternatives.

NEW PATIENT FAMILIES

$900 – This enrollment fee covers your family for one year from the month of enrollment. 

EXISTING PATIENTS

(the second year of enrollment and beyond)

$450 – per family per year
if any children are under 6 years old
$270 – per family per year
if all children are 6 years or older
 

After you are clear about our practice details, we can offer you a FREE 15-minute meet and greet appointment. You can come into our office, see where we are, and meet with one of the doctors about your health concerns for your family. (This is not a medical exam.)

  • To join our practice, fill out the one-page enrollment form.
    Download it now.
    (Or find it in your enrollment packet.)

  • Enclose a check or credit card number for your Secret Garden payment, and mail to us.

  • Upon receipt, we will enter you into our computer system and create your charts. Please allow 5 days from receipt before making your first appointment.

  • Lastly, call your previous pediatrician's office and have your child’s medical records sent to us, especially his or her immunization card.
