Insurance*
We are contracted providers for Blue Shield PPO, Blue Cross PPO, United Health Care, Guardian and Private Health Care Systems (PHCS). If you are a member of these plans we will bill your insurance and you are responsible only for your co-pay. For other insurance plans we will bill your insurance for you. Most PPOs will reimburse us as an out-of-network provider. You will be responsible for the difference.
HMOs are not reimbursed.
